Athletic Forms and Documents

Mountain View Christian Academy has two forms that are required annually for all athletes - the signed Athletic Handbook Agreement Sheet and a sports physical.

Please read the 2025-2026 Athletics Handbook before signing and returning the Athletic Handbook Agreement Sheet.

For the athletic physical, you may use the VHSL Physical Form (updated 2025) or another form that's provided by your child's doctor.

Both forms are due annually before the first day of practice for your child's sport.
